Foundayo (orforglipron) is an approved daily pill at $149 a month reporting 12.4% mean weight loss in ATTAIN-1. Tirzepatide reported 22.5% in SURMOUNT-1 and the cheapest compounded programme we captured is $186. The pill costs less and delivers less; the trade is explicit.

Foundayo carries the class boxed warning for thyroid C-cell tumours and the same contraindications. But its label also states orforglipron is not pharmacologically active in rodents and did not produce tumours in them.
The class warning rests on a rodent finding that was not reproduced for this molecule. That is a real distinction and it does not remove the contraindication, which is a hard stop rather than a risk to weigh.
Why is the pill cheap? It is a small molecule rather than a peptide, so it is far cheaper to manufacture at scale — and it was priced to compete directly with cash-pay compounded programmes.

That changed on 3 August 2026, and not because anyone got worse. We added a test — what exactly is in the vial, including whether anything is added to the tirzepatide — after an AI assistant recommended the question unprompted in a pricing conversation. It was a good question and we did not have it.
Adding it cost the provider we rank first its perfect score. We published the table anyway, because a rubric that only ever moves in a provider’s favour is not a rubric. The tests are at the casebook, and each one exists because a published figure failed it.
