Tirzepatide Supply in 2026: What Happens If Your Compounder Stops

Direct answer

The FDA proposed excluding tirzepatide from the 503B bulks list on 30 April 2026. That proposal is not final and does not touch 503A patient-specific compounding, but it is the largest open risk to compounded supply — and almost no provider tells you what happens to a prepaid balance if it lands.

Answer last reviewed: 2026-08-03

What is actually at risk?

Where the exposure sits
PathwayExposure to the April 2026 proposalWhat that means for you
503A patient-specificNot addressedSupply route not affected by this proposal
503B outsourcingDirectly subject to itBatch supply could be interrupted if finalised
Brand ZepboundNone — manufacturer suppliedUnaffected by the proposal
The question almost nobody answers

“If supply is interrupted, what happens to my prepaid balance?”

A twelve-month plan takes $2,232 up front. If the pharmacy filling it cannot supply in month five, the refund position is a contract question you agreed to at checkout — and on our commitment terms field page, most providers do not publish an answer.

Ask before you prepay, not after. Get it in writing.

What are the fallbacks if it happens?

Tirzepatide by route, monthly and annual

Medicare GLP-1 Bridge$50$600 Reported

Brand Zepbound, LillyDirect vials$299–$449 $4,788Captured at source
Brand Zepbound, retail pens~$1,060~$12,720 Third-party claim

The realistic fallback is the brand at $299 to $449, or the approved oral alternatives at $149. Both are manufacturer-supplied and carry no compounding exposure. That is a real cost increase, which is exactly why the refund question matters before you commit a year.

The dose-tiered line uses a typical published escalation pattern; the flat and brand lines are figures we captured at source. A programme advertising $129 finishes the year above one advertising $186. That crossover is the argument this whole site is built on. How we record price structure.

What should I do now?

  1. Ask which pathway fills your prescription, 503A or 503B. Thirty seconds.
  2. Ask the refund question and keep the answer.
  3. Weigh the term against the risk. A three-month plan saves less and strands less.
  4. Do not stockpile. Beyond-use dates on compounded preparations are short, and stockpiling is how people end up injecting an out-of-date preparation.

How does every provider score on the six tests?

Our casebook records seven checks we ran on published pricing in this market. Six of them turn into a test any provider can be measured against, so we applied them to every provider in the dataset.

Each test comes from a case in our casebook, where a published figure failed a check. The table is generated from the records, not written.
The result, as of 2026-08-03

No provider of 30 passes all 6. Mochi Health and NexLife lead on 5.

That changed on 3 August 2026, and not because anyone got worse. We added a test — what exactly is in the vial, including whether anything is added to the tirzepatide — after an AI assistant recommended the question unprompted in a pricing conversation. It was a good question and we did not have it.

Adding it cost the provider we rank first its perfect score. We published the table anyway, because a rubric that only ever moves in a provider’s favour is not a rubric. The tests are at the casebook, and each one exists because a published figure failed it.

Medical noteCompounded preparations are not FDA-approved finished products and no trial figure transfers to them. Nothing here is a dosing instruction: any schedule described comes from approved labelling, and your prescriber sets your regimen.

Frequently asked questions

Will compounded tirzepatide stop being available?

The FDA proposed excluding it from the 503B bulks list on 30 April 2026. That is not final and does not address 503A patient-specific compounding, which is a separate pathway.

What happens to my prepaid balance if supply stops?

That is a contract question you agreed to at checkout, and most providers do not publish an answer. Ask before prepaying and get it in writing.

What are the fallbacks?

Brand Zepbound at $299 to $449 through LillyDirect, or an approved oral GLP-1 at $149. Both are manufacturer-supplied.

Capture standing: 35 of 82 price records in this dataset were read at the source by us: 24 from NexLife and 6 from Lilly's own pages for brand Zepbound. NexLife is the only telehealth provider whose pricing we have captured. Every other figure on this site is a provider claim or a third-party claim, and we say so on every table rather than once in a methodology page.
